Congratulations on taking this first big step on your recovery journey. Our dedicated admissions staff is committed to making your experience as welcoming and simple as possible. Beginning with the initial conversation, we will gather screening information from you and quickly proceed to recommend the appropriate level of treatment care. Our goal is to work with you to map out the best possible treatment experience, which will allow for the most successful outcome.

DIRECT ADMISSION

If you are seeking treatment for yourself and you are NOT in an immediate life threatening situation, please contact Admissions at 918-664-4224. An Admissions representative will contact you within 72 hours to schedule your admissions screening appointment. Scheduling your screening appointment in advance serves as the most expedient and hassle-free way to begin your treatment process.

Intake assessments are conducted as scheduled from 8 a.m. to 5 p.m., Monday through Friday.

IF YOU ARE EXPERIENCING A LIFE THREATENING ADDICTION ISSUE, PLEASE CALL 911 AND/OR PROCEED IMMEDIATELY TO THE CLOSEST EMERGENCY CARE FACILITY.

REFERRAL ADMISSION

Referrals are often made by family members, physicians, treatment professionals, commercial insurance providers, social service agencies, employee assistance professionals and various state and federal agencies with whom we are contracted. If you are referring a client for Admissions, please complete the below referral admission documentation.

PAYMENT OPTIONS

Admission to 12&12 is available to every male and female adult 18+:

  • Private pay - Our admissions team will be happy to work with you in coordinating a payment plan that works with your budget.
  • Funded - We have federal, state, and private foundation grant funding to cover people who cannot pay for treatment. To see if you qualify for financial assistance, please complete the below Financial Assistance Program forms.

WHAT TO BRING

  • 7 pair of shorts
  • 7 pair of pants
  • 7 pair of socks
  • 7 pair of undergarments
  • 7 pair of long johns/thermal undergarments (in cold season)
  • Hats, gloves, scarves, ear muffs, hoodies and coats are acceptable (in cold season - MAY ONLY BE WORN OUTSIDE)
  • 7 long sleeve shirts (in cold season - MUST BE WORN UNDER SCRUBS)
  • Shoes or slippers (MUST BE WORN AT ALL TIMES WHEN OUT OF ROOM)
  • Toothbrush
  • Solid deodorant (no aerosol cans)
  • 1 Shampoo / 1 Conditioner
  • Makeup (Limit 2 items WITHOUT MIRROR)
  • Electric razor
  • 2 bath towels, 2 wash cloths
  • Brush and/or comb
  • Stationary
  • TB screening results
  • You must have all up to date non-narcotic prescription medication from home in the original bottle with legible prescription label in your name. You must bring enough medication for your stay at 12&12 Inc.
  • Additional items allowed include alarm clocks, radios, CD or MP3 players (only with headphones; nothing with recording capability or inappropriate pictures/videos will be allowed.)

Please DO NOT bring anymore clothing or items than what is listed. Scrubs will be provided by 12&12 during your stay in Detox.

If you are admitting to 12&12, Inc., no more than seven changes of clothing such as:

  • Jeans (if low rise, all body parts must be covered)
  • Tank tops (must follow the three finger rule)
  • Shorts, (no more than 1 inch above the knee)
  • Sweatsuits
  • If additional items over 7 changes are brought at any time, whether at admit or during your stay, they will not be accepted and will be placed in a tub and available for pick-up for 48 hours. If not picked up, all items will be donated or destroyed.
  • T-Shirts (no design or verbiage of a sexual nature, drugs, alcohol or violence)
  • Personal hygiene items, not containing alcohol
  • Bed linens and pillows are provided, but you may bring your own
  • Wash clothes and towels are provided but you may bring your own
  • You are responsible for doing your own laundry and should bring laundry soap/dryer sheets.
  • You may only bring plastic hangers
  • You may bring appropriate books, magazines, crossword puzzles, notebook, paper, pens, and/or pencils
  • Additional items allowed include alarm clocks, radios, CD or MP3 players (only with headphones; nothing with recording/capturing capability or inappropriate pictures/videos will be allowed)
  • You must have all up-to-date non-narcotic prescription medication from home in the original bottle with legible prescription label in your name. You must bring 30 days of medication unless it is a psychiatric medication, which would need to be a seven day supply.

If you are admitted to Transitional Living or VA-IOP, only the following additional items are allowed:

  • Cell Phones
  • Television
  • Stereo

WHAT NOT TO BRING

Items not to bring for all levels of care:
  • Halter tops, spaghetti strap shirts, muscle shirts, miniskirts, short shorts and tight pants
  • Pillows or stuffed animals
  • Hygiene items that contain alcohol- including but not limited to mouthwash, cologne and perfume
  • Fingernail polish or fingernail polish remover
  • No outside food or beverage may be brought into the facility
  • No weapons, including pocket knives. These items will be held in a locker and returned upon discharge
  • Any/All communication devices are prohibited (cell phones, smart watches, etc.)
  • No aerosol cans of any type
  • Electric clippers
  • Backpacks and purses (allowed in Transitional Living and VA-IOP, all others will be held in client's locker)
  • Cash, gift cards or other forms of money
